General Board Participant Forms

This page provides links to the most commonly used participant forms for the retirement and welfare plans administered by the General Board. Please contact the General Board, your plan sponsor or your annual conference if you need a form that is not listed. Also, please note (where indicated below) that some completed forms should be forwarded to your plan sponsor or annual conference, not sent directly to the General Board. Form Description
Authorization of General Board to Release Information To provide authorization to the General Board to release specific medical-related participant information to designated party(ies).
Before-Tax and After-Tax Contributions Agreement (UMPIP) To set up an agreement between the participant and plan sponsor/salary paying unit which specifies the amount/percent and the type of contribution the participant wishes to make to UMPIP. Return this completed, signed form to your plan sponsor/salary-paying unit.
Before-tax Contributions Election for the Horizon 401(k) Plan To set up an agreement between the participant and plan sponsor/salary paying unit which specifies the amount/percent and the type of contribution the participant wishes to make to the Horizon 401(k) Plan. Return this completed, signed form to your plan sponsor/salary-paying unit.
Chaplains Grant Fund Application To allow eligible clergypersons who have been endorsed by the United Methodist Endorsing Agency as a chaplain or pastoral counselor to apply for a grant from the Chaplains Supplemental Pension Grant Fund.
Claim Form for Survivor Benefits To request consideration from the General Board for a claim to death benefits.
Depository Agreement for Payments To provide authorization to the General Board to send payments directly to a participant's financial institution.
Designation of Beneficiary for Retirement and Welfare Plans for Participants To designate the beneficiary(ies) eligible to receive retirement and welfare plan benefits in the event of a participant's death under the terms of the applicable plan(s).
Designation of Beneficiary for Retirement and Welfare Plans for Surviving Spouse/Non-Spouse/Alternate Payee Accountholder To designate the beneficiary(ies) eligible to receive retirement and welfare plan benefits in the event of a surviving spouse, non-spouse or alternate payee's death under the terms of the applicable plan(s).
Election to Rescind the PDA Participants with Participant Directed Accounts (PDAs) may participate in LifeStage only if they rescind their PDAs. This form allows participants to rescind their PDAs.
403(b) Contribution Planner Input and Instructions (UMPIP) To provide the General Board with participant data for purposes of determining annual contributions limits for the plan year.
Hardship Loan Debit Agreement (UMPIP) To authorize electronic funds transfer for loan repayments to UMPIP.
Horizon 401(k) Plan Loan Debit Agreement To authorize electronic funds transfer for loan repayments to the Horizon 401(k) Plan for retired participants.
Incoming Rollover (UMPIP) To allow participants to roll over eligible rollover distribution monies that are acceptable to the General Board into UMPIP.
Incoming 403(b) Exchange/Transfer (UMPIP) To allow a participant to authorize a direct transfer of monies from a 403(b) plan with another financial institution to UMPIP.
Investment Election Form To notify the General Board of new elections or changes to participant investment elections for the defined contribution retirement plan accounts.
LifeStage Personal Investment Profile This form allows participants to set (or change) the variables of their LifeStage Personal Investment Profile.
Participant Information Release To allow a participant to authorize the General Board to release benefit plan information, excluding any medical information, to a specified individual.
Rollover to the Horizon 401(k) Plan To allow participants to roll over eligible rollover distribution monies that are acceptable to the General Board into the Horizon 401(k) Plan.
School Certificate (CPP) To provide the General Board specific information relative to an eligible child's full-time enrollment in school for the purpose of qualifying for educational benefits.
